A comprehensive treatment program combining transcranial magnetic stimulation (TMS) and self-compassion training: an intervention study for suicidal ideation in patients with depression

Background : Previous studies have shown that transcranial magnetic stimulation (TMS) and self-compassion training can effectively alleviate depression symptoms by regulating brain activity and enhancing individuals' positive self-perception and coping mechanisms. The purpose of this study is to explore the efficacy of TMS combined with self-compassion training in adolescents with depression who have suicidal ideation.The research hypothesis is that compared with TMS alone or self-compassion intervention alone, combining TMS and self-compassion training can intervene from both physically and psychologically, and more comprehensively reduce the suicidal ideation of adolescents with depression. Methods: Participants:The study will recruit 120 adolescents diagnosed with major depressive disorder according to DSM-5 criteria and experiencing suicidal ideation. Random Assignment:Participants will be randomly assigned to one of three groups: TMS with Self-Compassion, TMS Only, or Sham TMS with Self-Compassion. Interventions: TMS with Self-Compassion Group: Participants will receive 10Hz TMS treatment over the left DLPFC and engage in self-compassion training; TMS Only Group: Participants will undergo 10Hz TMS treatment over the left DLPFC without self-compassion training. Sham TMS with Self-Compassion Group: Participants will have the coil placed without magnetic stimulation and will participate in self-compassion training. The process includes pre-experimental assessments, a 4-6 week experimental phase with 5 sessions per week of 37.5-minute TMS treatments, and post-experimental assessments. Primary outcomes will be changes in depressive symptoms (HAMD-17 scale) and suicidal ideation. Secondary outcomes include quality of life and social functioning. Data will be analyzed using repeated measures ANOVA or mixed-effects models. Expected Results: It is anticipated that the TMS with Self-Compassion group will demonstrate superior outcomes in reducing depressive symptoms and suicidal ideation compared to the other groups, supporting the therapeutic potential of combined TMS and self-compassion training. Conclusion: This research aims to provide new perspectives and methods for the treatment of depression, offering a more comprehensive treatment plan for patients by combining TMS therapy with self-compassion training.
